Registration of a change in the surname ending
Based on the application of every citizen of the Republic of Azerbaijan who has reached the age of 18, the ending of their existing surname may be changed. For individuals under the age of 18, the change in the ending of their surname is carried out upon the application of their parents or other legal representatives (adoptive parents, guardians, or custodians).
When both parents’ surname endings are changed, the surname endings of their children under 18 are also changed accordingly. The ending of the surname of a child who has reached the age of 10 may only be changed with the child’s consent.
Required documents:
- Application for the change of surname ending;
- Identity document of the applicant;
- Previously issued certificate(s) on the registration of civil status acts;
- State fee: 20 (twenty) US dollars.
